Our comapany has introduced an Ex employee satisfaction survey to find out new ways to make the company a better work place and also to retain its existing employee.
We have developed this form for this purpose.

First of all it is a nice draft for those who are separating from the organization.
Now I have some question on your posting:
1) How will you analyse it ?? Have you plan some thing?
2) What is the frequency time of presenting and discussing the outcome of the survey with top management.
3) Who are the steering committee member who will resolve the major issue, if any.
You may add on 100 such questions but in case you do not strive to implement the outcome of survey , the entire exercise will be futile.
however, good initiative.

A very good attempt.
Yes, employees will be more likely to put down on paper what they might not have liked to discuss in person, so comments will be franker and more relevant.
However, you must followup and make sure, as best you can, that the form is filled in by the ex-employee within a week of resigning.
Also, as Predipta said, work out a policy that will let you discuss the comments made with the supervisor directly involved and get his counter comments also.
Make a separate file of these forms and mark the trends which appear - if there are certain areas which are constantly being highlighted, take it up with your superiors and the relevant supervisory staff to see how matters could be improved.
