I am working in SSI Industry where HR. Policy of Leave application is frame and since Feb-13, that any kind of leave, should be intimated to their Immediate boss/HOD & HR Within 5 days after their leave days completion, failing to this their CL/PL/COFF will not be adjusted against this leave, is this policy viable.

A couple of things you can do:
a) Make a clear leave policy covering every scenario and a clear guidelines as to what happens in each of them. Take care to make a balanced policy - particularly, not too draconian.
b) Talk to senior managers, tell them about the policy and ask for their help in implementing it. Ask them to communicate it to their teams.
c) Circulate the policy in the company. If possible, meet teams and tell them why such a policy is required and ask for their cooperation.
d) There would still be defaulters. Be strict with them. Make examples.
e) Ask for suggestions. Keep feedback/communication channel open.
