Dear Nisha,
I tried to access your questionnaire to see how it is structured and what information you are trying to collect. However, I could not go beyond page 1 without answering the questions. Asking personal information at the outset is a really turn off for people to help you.
You also should include a little note at the outset about where you are pursuing the PhD, the purpose of collecting the information, how it will be used, and how it will benefit those that answer the questionnaire. You also need to put a note about the confidentiality of the information that the respondents provide, etc.
