Request For The Format Of Pay Slip - Xls Download

Sir ,
This is Govindu, I am working in a Pvt Ltd company from the past 4 years my payment is through cash and I was not provided with a payslip.
At present I have an offer in an MNC where I was asked to submit my present payslip. I would request you to send me the payslip format and if also their exists any other alternative. Expecting a speedy response from you.

I want to continue working in this company.So how can i ask my HR for the salary slips.
