Please clarify what you want to know so that seniors can help you, with regards to PF deductions it is 12% on the Basic Pay from both the sides i.e. Employee and Employer, with regards to ESI the deductions are on Gross Salary, Employees Contribution is 1.75% and Employers contribution is 4.75%. If you require further help please write in detail

What % you are looking for, please elaborate so as to give you appropriate answer. With regards to PF & ESI deduction does not vary from state to state as it is centrally governed. The Contribution towards PF is 12% on Basic Salary for Both Employee and Employer even an admin Charges are also applicable which employer has to contribute which is approx. 1.61%.
The contribution towards ESIC is 1.75% on Gross Salary for Employee and 4.75% for Employer.
The Professional Tax varies from state to state.

TWO KIND OF PF:

2)UNLIMTED-what ever the basic X12%=xy

There is no format of change in company's name, if you want to disclose to clients it can be drafted as circular
Draft Circular
This is to keep you informed that our company namely xyz has changed its name to ABC with effect from 1/11/13. All the directors/partners and working style will remain same. No change other than name is incorporated.
Any orders pending in the previous name will be completed and the invoice will be raised in the old name and the payment be made in the old name on or before 31/12/13.
All new orders will be entertained in the new name only.
You can add/edit or draft the circular as per your need
