I joined a pvt ltd company in sep 2016 and I am not confirmed yet. I made five unschedule leaves during the period. My manager has given final warning for my act. Now I had a health issue and I took three days leave and I only sent a sms about my situation. Are they going to terminate my services with the organisation?

See there are many issues here. First the labour laws entitle you some leaves irrespective of your company policy. First see that whether you trans-graced it or not. As you have not mentioned the nature of business, kindly see the website below and see your leave entitlement as per manufacturing or non manufacturing concern. Also see there leaves under Shops and establishment act of your state. Second if your health was that bad that no reasonable person would go to work in that condition, they cannot terminate you.

The company can terminate you without reason during probation.
They do not need to confirm you, it's a choice lying entirely with them
In general comaonies frown on leaves during probation and definitely don't like unscheduled and unapproved leave. It is natural that the Company will therefore be unhappy. It is generally considered that if an employee does not behave properly during probation, it's unlikely to be a good employee during confirmed / permanent status.
Sending out just an SMS during absence, not even calling and telling the manager you have a problem is generally considered unacceptable
If they decide to terminate there is very little you can do
I suggest you go and apologise to your manager and explain the situation
