Please find the following info on ESIC, hope it will be useful to you.
Till last month ESIC eligibility is upto 15000/-Gross Salary.It was extended from 15000 to 21000 from 01 Jan 2017 onward . That means who are under 21000 gross salary will cover under esic act.

The Employees' State Insurance Corporation (ESIC) today raised the monthly wage limit to Rs 21,000, from the existing Rs 15,000, for coverage.
The ESIC board also decided to give an option to existing insured persons (IP) to continue their membership even if their salary raises the limit of Rs 21,000 per month.
Currently, all those insured (IP) under the ESIC scheme lose their ESI membership as well as that of the insurance cover if their salary/wage raises over the limit.
Labour Minister is the Chairman of the ESIC Board and he said "ESIC has raised the threshold wage limit from Rs 15,000 to Rs 21,000," after the board meeting of ESIC in New Delhi. Both the decisions will be implemented from October 1, 2016.
This move of raising the limit will help bring in an additional 50 lakh members to ESIC scheme.
Currently, ESIC Scheme has 2.6 crore insured persons (IP), which covers over 10 crore people, assuming four members of a family (Dependents).
The minister also said there is a plan to increase the wage threshold for retirement fund body EPFO subscribers and it may be considered in the next meeting of the Central Board of Trustees (CBT).
At present, the wage threshold is Rs 15,000 per month for coverage under its (ESI) social security scheme.
