I work in a BPO and we are planning to start Training and Development division separately to continuously have training programs for various employees in various topics. The first and foremost being in communication,personality development etc. Can anyone help me with a questionnaire or any other form which will let me assess the training needs and rate the employees so that it becomes easy for us to choose who has to be trained on what topic.
All or any of your views will be greatly appreciated and helpful.

Choosing who to train, on what subjects is a big issue. You have probably already done this, but I would suggest taking the following steps:
1) Define the core skills required (and the level required) for each key job
2) Map the skills levels to your training programmes (there may be gaps where you do not have training yet)
3) Assess your staff to see what their current level actually is for these skills - have their managers confirm this assessment
4) Use the resulting skills gap, mapped against the training programmes (from step 2) to produce your training plans. These can be for each individual, but, rolled up, will also be able to show you the training needs across the organisation.
