I would like to suggest you not to go for any templates. First please study you company policy to spend on the business travelling of employees. Then study more about the places they visit for business purposes. Then you study the designation of the employees who travel.
Now the latest HR policy on this is company fix a particular amount on each designated cadre / grade. Not on actuals because all employees will have a scope to miss use this facility. Try to get some dummy bills like travelling, entertaining some business people (this also depends on the Grade of the employees).
The company will give a fix amount (after good survey of all aspects about the business trip and how much does it cost).
If the stay in five star Hotel or in Lodge that's not matter for the company.
If not all arrangements should be done by company without involving them.
Think on this aspect. Then decide according to the company rules & regulations.

At the outset you need to define the need/ Objective and Scope of the same. Further you divide the countries/ cities based on cost of living into two/ three categories and accordingly define the daily allowance (may be ranging from USD750 per day - USD200) this will cover all the expenses like lodging, boarding etc. Further in case it's a sponsored trip you can have provision of 50%/25% of actual D.A. Local conveyance, Visa charges, Air tickets, Medical Insurance needs to be defined at actual and for higher officials entertainment allowance could be added.
In additon you can have clause of contingency expenses subject to submission of bills to meet out any unforeseen expenses.
