Dear Sravaniy
I am really getting surprised over your question because, as an HR EXECUTIVE, why you are asking this question.
You advise the management, those who are getting Rs.15k & belowRs.15k, to be covered under the ESI SCHEME.That is your right also

Hi Sravaniy,
As stated by many seniors here, any employee whose salary is less than 15Kpm is liable for the ESIC even if willing to take it or not. It's statutory by the Govt. of India. 1.45% of your salary is deducted from your salary & 4.75% would be contributed by your company.
All the ESIC work is online now & you personally visit ESIC office only when you have to get the photographs clicked & finger prints scan.
Also, like you mentioned that you need ESIC for family purpose so you would need to take along the family members along to the office, whom you want under this scheme, for the ESIC formalities. You would also need to take an affidavit mentioning that the your family is dependent on you.
Hope this helps! Also, being HR you must be clear with al these things as you shall be responsible to the Audit people & employees, if enquired.
Best Wishes,
