Salary of Rs25,000 and i need ESI is there any possibility?

If i am getting a salary of Rs25,000 and i need ESI is there any possibilities to get ESI benefits anybody kindly suggest

Hello sir,
Iam working in IT sectors company. In my company 1 employee whose salary is Rs 12561 PM , and he was not interested in deduction of PF and ESIC.
So , in this possible or if possible then what is the procedure or which form i fill for this.

Yes it is possible. But Management is ready for this. He need to fill Form No.11 (Declaration Form). In this form he need to declared that he is not getting PF from his Previous Employer.
