Dear Rusky,
You can Claim your EPF by simply Filling up form no 19 & 10C, which can be obtained from local PF office or can be downloaded from. www.epfo.org.
Pf can be claimed only after 59 days from the leavin date.
The form obtained must be signed /attested by the last employer before submission.
Thanks,
Rahul
