Seems a good one for me too. You have covered most of the points. Also you can check for any confidential information that would be provided to the employee which he would not be expected to leak. Confidential information can also include one's salary.

I don't know the size of your company, but to ensure uniformity we used our checklist and produced a 15 minute inhouse video covering all of the HR subjects. This has been very beneficial. It is a little scary when you start the project but using Power Point you can prepare the slides in outline format and then script it out of the employee handbook. It will take a few tries but it will be worth it.
I don't see anything about a "progressive disciplenary" policy to enforce the policies and procedures outlined in the orientation.

This is indeed a good one. But I think it can be made better by adding some points of check list in Quality and Admin section as well.
You can also add a section of System Admin / IT hardware Department, which take care of all the hardwares /softwares /System provided to the employee. You can make some fields in it and add on that section as a check list.
Probably then your form would be a complete one.
