Dear Colleagues,
Can anyone help me in creating a one page bi-annual performance review form. We are planning to start this system and will provide marks to employees. Based on these marks (twice in a financial year) we will be incrementing employees.
We are a garment (uniforms) manufacturing company in Mumbai with employee base on payroll of around 100 employees.

I have created a Sample appraisal format in which I have included some point to the best of my knowledge. I am attaching the same in this post. Please provide your inputs on the same. I am trying to keep a transparent review system in my company so at every end of the financial year we will be able to justify appraisals.
Secondly, all unnecessary favoritism will be eradicated as year round performance will be available on paper and therefore everything will be black & white.
Please suggest.
