Please Issue, First, a Show-Cause Notice detialing instances of Poor/ Under Work-Perfomance ( Date, Time, Place & Performance per se) informing the Under Perfomer about Underperofmance which amount to Act of Misconduct as per Service Rules applicable for which s/he is liable for Disciplinary Action (Proportionate Punishment included) , Give reasonable time to submit his/her Written Explanation as to why Discilinary Action need not/ should not be taken for the alleged Act of Misconduct; thereafter
Appoint a Capable Inquiry Officer to ascertain the Facts/Truth about the allegation as per Procedures laid down and in accordance with Priniciples of Natural Justice,; Let the Punishing Authority apply his/her Mind to te Findings of the Inquiry and then only you Need a Draft / Sample Warning Letter.

the word 'poor performance' is a very indefinite, indeterminate, and ambiguous. Unless you have specified performance level, for the individual concerned , which can be measured/ quantitative, your issue of warning letter does not make any sense. If for the staff for failure in usual work / or mistakes / or less production volume. I am giving below a draft, which you may modify it suitable to your situation:-
"This refers to the performance for the last one week. (quantify with dates and timings and less commitment) The above act of yours is a serious misconduct / and affecting the production, discipline inside the shop floor.
Please note that we will be watching your performance for the next fifteen days. Unless your performance is improved, we will be left with no alternative than to initiate further disciplinary action as deemed fit, including appropriate wage / salary deduction.
Considering all extenuating circumstances, and considering the fact and strong belief we have, on you, that your performance will come to our expectation / target within the next fifteen days, we let you off with a ‘WARNING’ now. Please note that any repetition of similar misconduct / poor performance on your end in future will be viewed seriously."
