Hi KR,
You need to appy in prescribed format alongwith the data like balance sheet, company registration under company laws, directors details and employee detail.
However my suggestion is to take a help of PF consultant for this. There will be definately back dated payment, copmany may have to pay.

Dear KR Mathur ji,
Your company was liable to obtain PF code and make the remittence of PF dues from the date your company had 20 and more employees including of contract labours irrespective of their salary limit. Your company's liability of PF dues with interest and damages start from this date. The same will be calculated on the basis of various records & registers, balance sheets, books of accounts, account ledger etc.
