during OHSAS 18001:2007 audit , the auditors asked for to add behavior based hazard on RISK register,
please Share Behavior based hazard & injury(as attached format)... also plz find attached format for HIRA its for paint shop...(example)

Upon your request let me try to share my thoughts about the concern.
Please go through the attached sheet. Read my comments in red & blue.
According to me, this HIRA having drastic scope of improvements. This seems like prepared for getting certifications... Just for the name sake of HIRA...
Mr. Gaurav started this thread in 2012 and I am sure its not going to help him for sure.
You let us know whether this suggestion in helping to you or still needs some more clarifications.
Come back to the forum with your thoughts/concerns... All the very best...

Dear Dipil

I need more clarification for the above subject. I here by attached a risk assessment of the "grinding" activity. Already we mentioned in the RA, the control measures in order to prevent the hazards, but some times people they will think this is a small job so no need to required the PPE and after they got injury. In such situation, human element is also considered and the OHSAS says that;

“Human Element” in risk assessment : OHSAS 18001



OH & S hazards are not limited to faulty equipment and hazardous materials. They also involve the human element – how people behave and why they behave as they do.



Reference OHSAS:18001 section 4.3.1 : necessitates human behavior, capabilities and other human factors be taken into account during hazard identification / risk assessment



OHSAS suggests that when considering hazard identification, one should consider à

The nature of job (work place layout, operator information, work load, physical work, work patterns)

The environment (heat, lighting, noise, air quality)

Human behavior (temperament, habits, attitudes)

Physical capabilities (cognition, attention)

Physiological capabilities (biomechanical, anthropometrics / physical variation of people)



Considering these factors, and their interactions, can help determine human element and appropriate controls to be put in place.
