Sharing as received:
Please find attached file herewith illustrating an incident (Plant/location-unknown)during material handling by hydra FYI & sharing with all concerned.
Contributing Factors :
1. Operator was listening to Music from Mobile Ear Phone while operating.
2. Two other persons were traveling in Hydra distracting the operator.
3. Use of Hydra to lift & shift Long and heavy Loads.
4. Guy rope in the victim's side was less than 1 meter.
5. Single leg Hitch was used to sling long loads.

Dear All,
Hope by seeing the photographs people it would be a mega construction project only. It clearly indicates the the safety awareness and the safety management system at their site.
Risk is associated with all jobs, It can be reduced / controlled only by doing JSA job safety analysis / HIRA - Hazardous Identification and Risk Assessment.

Our safety trainings should give emphasis to all these aspect also... Awareness and visible commitment towards safety from top management/govt. officials is the only cure...

it is completely an attitudinal problem. People take everything for granted. This incident is purely due to negligence. Wisemen will learn from others' mistake, fools will never learn from even their own mistakes. How many of us want to be wise, choise is yours.

Its a real eye opener incident. I agreed with Mr. V Balaji that people take every thing granted. Always remembering one qoute from my Safety Leader that "SAFETY IS JUST A COMMON SENSE". Most of the incidents happening due to negligence only.

Dear Vijay,
Exactly you said right vijay ''We are paying much valuable human life to learn safety''.
Everybody knows what we wanna do??? what is rule everything... very little example how many of then using HELMET while drove 2 wheeler? Most of them using helmets but they are not ON his chin strip's while using the helmet
See the live example of mine (I shame on myself to say this in my helmet which i have using in India its have chin strap but its damaged i know if i fall surely it will fly away from my head but still i am using it????) I am sure will change it next time!!!

Dear dipil,
great job.
in our shipyard also movement of hydra is very high. hydra operator got the habit of listening to music while driving hydra and also talking on the mobile phones. i have posted your incident on notice board and also instructed security not to allow any hydra operator to take the phone inside. We have SOP for goods movement and we are giving training to workers and hydra operator to follow this SOP.
