Hello Astha,
I presume you are asking about training participant feedback forms. When designing this kind of feedback form, I use my MICE principle. Using this principle ensures that I cover all of the basic areas in my feedback form.
These areas that the form needs to cover are:
Materials – quality of any trainee guides, handouts etc
Instructor – quality of the instruction and adherence to adult learning principles
Content – relevance of content to the stated learning outcomes
Environment – quality of the learning environment (seating, temperature etc)
I include a number of questions using a Likert rating scale from 1 to 5 so that I can draw some numerical conclusions. I also include a number of free text areas in which the participant can write whatever they like.

Hi astha ... u know it's very easy to design a feebback form when u know what ur objectives are . I mean it may be a Pre training feedback form or after training feedback form . Or Training effectiveness form .
So just define ur objectives . N then work on it . Feedback from includes
