Thak you for posting the questionnaire and giving some details about you. Is it a full-time course? Have you got a tutor guide and has the guide seen your questionnaire?
Expecting people to download, answer, save and then send it back to you is not the best way to elicit positive responses. Kindly use a web-based survey tool. Also, it is advisable to ask personal questions at the end. Please go through the questionnaire, read it aloud, and see if the statements and the scales chosen make sense or do they need altering to improve clarity (for example, The company provides WLB policies...). Also, what do you mean by "amount of notice" for a "yes" or "No" response.
