Nominee has to file the claim in FORM 20 and needs the following documents :

• EPF Claim FORM 20
• Affidavit of Dependents in favour of Nominee
• ID Proof of Dependents & Nominee
• Declaration of nominee
• Bank Details of Nominee

Kindly arrange all these before filing EPF death claim.
