Hi Maulik,
In addition to Mitali's format where the outcome of an appraisal is positive; there are times when the opposite is true.
Here is a format when that is the case:
Dear xxxxxx,
On DD/MM/YYYY, you attended a Performance and Appraisal review meeting.
After careful consideration of the discussions, I regret to inform you that your performance to date has been below expectations.
The reasons for this are (insert reasons).
We expect your performance to now improve substantially and will monitor your progress over the next 3 months.
If there is no improvement you may be subject to disciplinary action being taken against you, which may lead to the termination of your employment.
As a result of this decision you will not be awarded an increase in your gross salary.
If you feel that you require further mentoring/training then please discuss this in the first instance with your manager.
This letter will remain in your personnel file and will be referred to at your next appraisal.
Should you wish to discus this decision feel free to contact me, otherwise please acknowledge this letter, by signing and dating the enclosed copy.
Yours Sincerely,
(Name)
(Designation)
I hope this is of use to you.
