If you are working in an HRIS project for a company, a readymade questionnaire would be of no use. You will have to sit with your project guide and understand the scope of the project.
There are a lot of parameters that should go into making an HRIS q'aire.

- Size of the organisation
- Departments that will be covered in the sample size
- Stage in which the HRIS exists: Has it just been introduced and employees are yet to gain familiarity for working on it? OR has it been around for a few years and has started helping in taking strategic decisions?
- The various HRIS modules that are in use, the most useful ones, etc.
- The modules that are the least used: the reasons
