Some Adjustments in that form....as of my knowledge....
what is diferance between 6th and 7th columns,fathers's mother's:?::icon6:
a)Just u mention family details that's all instead that detailed 6th coloumn
b)Just u add 3 more column as (1) Marrital status insted of name the spouse/son/doughter...(2)Expected salary(3)Experience details/salary for further referance (4) names/Contacts of the Referred candidate (5)Enclosed Documets (DOB proof V.IMP as a labour act)
C)In the 10th column, u can use the Colums of that Excel sheet instead of rows to cover the space.

Thank you so much.
Again as I eralier mentioned - this is not for people at executive levels, with whom we can have a form for mentioning expected salary. They are raw hands, and salary is already fixed.
Family: Mother and Fathers details are important. 1 - you let the parents know that we are concerne about the safety and 2 ) Yes - we need to know about the family background of the village girl who is hired.
Experience details: They dont have experience :-) Raw hands
I will incorporate DOB proof.
Marital status - we can include. But definitely need the name of the spouse and their occupation too. It matters in this industry - esp with unskilled labor.
