Very sad indeed. Regular safety and awareness training should be implemented by all companies, even more so where employees work in hazardous areas with electrical safety requirements. Unfortunately L & D seem to be the first casualties of any budget cuts.

THe accident clearly indicate that there was huge loop hole in their safety system. The systematic RISK ANALYSIS would have been done to identify all such possible dangers and specific preventive measures could have implemented before startup the trial in the new plant.
