Since it is hospitality industry, you choose the following criteria. Please note that in this industry customer is the king and we cannot dispute this with any logic. Hence you need to take inputs from the customer in a non intrusive way(Not by asking him to fill an elaborate form or so) of his experience with your service.
1: Any incident that is positive and appreciated in words by the customer on any of our staff(Lowest level to the highest) can be one of the key parameter.
2: When a customer leaves(For eg if it is a hotel) he is normally asked to rate the service on certain parameters and in some hotels(My personal experience) they even ask any particular service like reception or checkin Cleaning the room or room service or housekeeping you liked. If a customer is happy he will definitely mention it. Then you can trace the name of the person and reward him.
3. Definitely service/hospitality industry needs to innovate itself. Hence any innovative method/product/service that your staff suggests and he owns this and drives it towards customer delight should be rewarded by employee of month.
4: You can also take parameters like attendance, punctuality, gromming (as mentioned in the excel somebody gave) etc as additional criteria for your selection.
The challenge for the organisation is to keep this award going on month after month. It should not become a pain where you have to select somebody because there is an award. Then people will not strive for this. There should be an acceptability among your staff that this person really deserve the award. Hence please plan for sustenance and forecast how you will run this program month after month. Once you set the ball rolling you need to keep it going :-). May be you will make the criteria tougher as you go on.

then a track should be kept of each and every activities of all the employees actions.
i would sugest that u m,ake EOM from each department. so u will gat more people to motivate.
and its not necessary that u connect all the actions of employee with monetary benefit of company.
if theres anything good or extraordinary thong done by him then ask the HOD of that deprtment to maintin it.
acknowledge him in front of everyone!

productivity ( both quantity & quality of work) & consistency
Handled additional responsibilities in same department or across other functions
new initiatives client feedback
other criteria like team player, attitude, mentoring, process improvements, attendance can also be factored.

1. Ok then prepare a one page templete which consists of five questions and ask each of your department head to fill it at quarterly. Based on all their comments choose the nominee for Best employee award.
Herewith i have attached a templete that i use in my company for Pier recognition award.
2. If this doesnt work then a simple way to execute this is --- Let each department head choose a member from their team(Mentioning the reason for nominating the nominee name and their key role played) and mail their name to the HR. Now HR and Management can decide on it.
3. If this doesnt work then ---- Quarterly call everyone for a meeting(Including management) and let each member convey what they have accomplished for the last 3months(Give 5min each). Let management announce the name.
4. If this doesnt work ---- then ask your employees to send Time sheet to the HR on monthly basis which includes the synopsis of the work that they have performed for the entire month. Based on that choose the Best employee.
5. If this doesnt work then --- HR should do a survey to find out the star performers either by interacting with the Department heads or by interacting with the employees itself. This includes both performance as well as character. HR can easily come to an conclusion on whom to nominate.
The most important thing is that HR should always be ready with an answer to face the questions from the employees because everyone assume that they have done their best and they are the best. Everyone will be curious to know on what grounds you have chosed a nominee and why not their name has been included. When we choose an employee as an Best employee then we should be in a position to let others know how is this particular employee different/best/unique from others as well as why was he/she eligible for this award.

On monthly basis get all the required inputs from each departmental head( use official email - avoid verbal communication) and as a HR evaluate/screen on some grounds and finally call everyone for a meeting to announce the award.
