Ajay Kumar - An 'A' performer and you still can't write out your own appraisal? And moreover, you want to 'impress' your management???
In my opinion, factual representation of facts is more conducive to appraisals then the 'impressionable' quotient we try to achieve.

Dear Ajay,
Please cite incident based data for each of the factor. Avoid open statements, such as "I am great at leadership skills". Even if you have to mention recommendations, quote who said it, when, where and why.
Here's a similar discussion we had just a few days back. Year-End-Review Dinesh Divakar had mentioned how to write a self-appraisal. It might help you to think better.

Hi, Be straight,be factual.Your superiors always know more,they know what you are.Best course(difficult to follow)is to avoid stereotyped observations,on one’s performance.

By now you must have been very conversant with the meaning, and purpose of Performance Appraisals.
On each of the attributes, you have to state, where you stand, based on the performance during the year. They should be true to the facts. A company would always look whether your continued good performance had been a cause to get more revenue. They count on R.O.I., First have a clear understanding of all the attributes you have to answer. Do try to impress by performance and not by gullible words in the performance appraisal. All the best.
