Hi Swapna
In your organisation is there any standard format for salary slip. Based on the management structure of Pay Slip will change. If you can get from your company then it will be fine.
There are more of deductions like PT, PF. PT varies from state to state based on that only Salary Slip can be prepared.
Even Gross salary also required to prepare Salary Slip

Hi Swapna
I can understand your concern.
Let me breif about some facts.
As your salary exceeds 15000/- there is no Any ESI Deduction
As your company has not registered with PF so No PF Deduction.
If your company has registered with PT then your PT will be Rs. 130/- (PT amount is Rs. 130/- for salaries 15000/- till 25000/-) if not registered then No PT also
For TDS contributions there are various calculations. As you are getting salary of Rs. 22000/- in cash then nothing to be worried. You will not be entitled for TDS.
If you want to calclate TDS also then there are lot of exemptions like HRA, Conveyance Allowance, medical Allowance. If you consider those then you will no fall under TDS Clause.
If required just get salary certificate and tell the new employer as there is no any standard Pay Slip in that Organisation.
