For the review of a probationer, there is no universal form. It will vary from one designation to another and also from one level to another. You need to design it on your own.
The common attributes for the evaluation could be:
a) Team spirit
b) Learning attitude
c) Speed of learning
d) Initiative
e) Adaptability (responsiveness to the suggestions)
I have not included communication skills in the above list because these you must have checked while the recruitment itself.
To design the review form, take the help of the interview evaluation form that was used for the recruitment of the candidate. Additionally, you may check the duties done by the probationer. What was expected from him/her, did he/she do it?
