I need a Format of Maternity Leave as per Delhi State Govt. Please help me on this.

Dear Sandhya,
Maternity Act is applicable to complete nation and it is not statewise. Secondly there is no specified format for application of Maternity Leave, you may give an written application to your HR for the same.
