I have some basic structure in mind. I shall be happy and thankful if any of our forum members can show the format followed in their respective companies (even if it is general it is welcome).

Hi SubramanianJ,
You took a long time to respond.
Anyway,Good to hear from you. I have gone through the formats you have designed, they are quite impressive. Although I would like to offer you some suggestions;
For PL- You can include the LTA also in the form, if the company have a policy of providing LTA. Once a year when LTA is claimed by the employee they ask for PL also, it would be good to include that.
For CL- This format is very comprehensive :D , one change you should do is put the column Employee signature at the bottom right corner.
I really appreciate your efforts.

I once again apologise for the delay in replying to your earlier communication.
I thank you very much for your excellent feedback.
Regarding, LTA our company does not have one at present. But as suggested by you we have to take in to account all future requirements when preparing formats and formulating rules. I shall surely do as suggested by you.
Regarding CL too, I shall do as pointed by you.

can u pl tell me in detail the type of leave applicable in a firm
as per my knowledge there is sl, cl & pl. Are there any more leave apart from these.
Also is preferential leave and annual leaves are the same.
in our firm we have total 48 days leave out of which i availed only six leave can i avail encashment for the remaining 40 days leave pl advise
