As i required to give a warning letter for our security supervisor who is alleged for unauthorise absenteeism, poor performance, found on duty in drunken condition Please help me for this.

Any Body can suggest me how to give the warning letter to our employee. He committed the following mistakes.
1) he came to duty and done the punch in bio-metric system and after one hour he left from duty without any intimation to supervisor or hr.
2) And he came to factory by evening and done the out punch.

2.Mention of any available Standing orders or HR Manual clause.
3.Mentioning the seriousness of the misconduct and its effect on the rest employees.
4.Mentioning the non repetition clause if he is let off this time.
There are erring employees always in any set up,HR has to be educating them in general about these kind of misconducts and the adverse effects of them. It can be also in employee induction or other awareness programs on Dos and Don't s,HR manual or Standing orders etc.
