Dear Venkata Ramana,
Initially what is your present position in the organization? Which is the company asking you to continue duty for 16 hours a day? It is exploiting you and in future the Management would be in trouble if they continue like this.
You need to speak to the HR team on your concern, get their approval in writing explaining your inability to continue the job.
I do not think there would be an objection for relieving you on medical grounds.

Dear Venkat,
Whatever your position in the organization, since it is no more possible to continue in the job under the health condition you mentioned only you have decided to quit. So the only question is how to make the exit process smooth both cost-wise and tactically since your total service is just 4 months only in which you were already on LOP for a month if the presumption for the last month's salary hold is correct. I endorse the suggestion of Mr.V.V.K.Patnaik. Better politely approach the top management for waiver of notice and immediate relief on humanitarian consideration of your health condition based on medical evidence as a special case.

Dear Venkat,
It is need to understand why your salary is on hold. You made an application to HR to release the wages for the last month and wait till they do. You have already made the mind to quit the job.
If payment is not made then lodge a complaint in labour office for non-payment of wages.

Dear colleague,
You want to resign on medical grounds but unable to fulfil notice period condition.
Your objective is to get smooth separation and get due payments. So try to earn sympathy by explaining your inability to continue on health grounds to the top person in persuasive manner.
You have limited options considering your only 4 month's service and disabling health conditions.
