What is the difference between Form A & B for Maternity Process. Our office is in Gurgaon, Haryana so for Haryana which form should be required to download.

Hi,
1. Form A for muster roll .
2. Form B Medical certificate stating her pregnancy from authorized medical practitioner.
3. Form C Death certificate ( During the pregnancy)
4. Form D In case delivery miscarriage.
5. Form F Receipt of Maternity benefit.
6. Form E Notice under section 6 of the Maternity Benefit Act for proceeding on Maternity leave.
