I have been conducting Exit Interviews from last four years and have got feedback on various odd issues- some times not even thought about

One ground rule to that is you should try and make the session less formal and start talking about the new job, location and family slowly steering your question to the good points a person felt while he was associated with the organization and whether he would like to consider if the opportunity arises in near future.

Then telling him the importance of his feedback ask him informally what he/she felt needs attention/improvement in the organization. Convince him that his feedback holds great importance and will give us the opportunity to answer the issues. Like this some times you also get to know the general feel of the work culture of the company- points which are a matter of concern

Last but not the least we also ask the leaving employee to fill the Exit Interview form- where besides the informal understanding that you have gained she/ he can write his thoughts.
