We have lost our Pf's allotment letter in a fire accident recently.
I want to reissue Pf's allotment letter once agn from PF dept.
So i want to know that is there any format for this letter to reissue.
If you have then kindly share with me asap.
I would appreciate this help.

Dear Dr. Vijay,
Greetings for the day,
First you approach to the concerned EPFO with the following documents and request them to reissue the duplicate one :
1) Covering letter dealt the whole situation for the loss.
2) Copy of form-5 (Return of ownership) in duplicate.
3) List of the Board of Directors with their correspondence address.
4) Memorandum of article of association.
5) Bank Details.
6) Latest paid EPFO challan (Photocopy).
7) Report of Police/Fire dept.
All the above cited document must be attested by the owner/ authorised signatory and submit the same to Regional PF Commissioner to the concerned EPFO duly acknowledge the same .
