DEAR HRs!! Please suggest me in this Issue

One of our employees has joined for our organization in the month of march 2013, he has been allotted ESI Number on behalf of the organization. ESI contributions have been paid for him for the months of March, April and for the month of May his contributions remained nil. That employee left the organization at end of May month and joined for new organization, there he has been allotted another ESI number and contributions are being remitted from the June month to October 2013.

Now the issue is; his daughter become sick and she is needed super specialty treatment in ESI Hospital. He approached ESI super specialty hospital for treatment, but they refused to give super specialty treatment to his daughter because he is having less contributory service in last ESI contribution period.

For getting eligibility for super specialty treatment IP should have contributed not less than six months in the last contribution period for dependants.

1) Employee is having more than six months of contributory service by adding contributory details of both employers. But how to merge the both IP numbers for obtaining eligibility? Whether it is possible or not?

2) If possible whom to be approached and what is the procedure?

3) If the mentioned dependant is needed very emergency treatment can we obtain a special permission for ESI Super specialty treatment from any other higher ESI authorities?

4) In above possibility whom to be approached and what is the procedure?

5) What are the measures to be taken to prevent arising of this kind of cases?

In my opinion, the employee may visit the appropriate Branch Office of ESIC (present Branch Office) alongwith old Identity Card as well as copy of Declaration form submitted to obtain old insurance number and also copy of Return of Contributions submitted for the previous contribution period. He may discuss the matter with the Branch Office Manager and request for making his dependants eligible for super-speciality treatment.
In order to avoid such like situation in future, it is essential that the employees disclose their old Insurance Number and Identity Card,if any, issued by ESIC at the time of joining new jobs/establishment.
