Form is good however, you can add some more points in it for clarification and future analysis like -
1. Is this a new position or a backfill position
2. can anybody from the department / organization can be promoted or transferred to do the job
3. whom the person will report / reporting structure
4. If this is a contractual position then for how many months / years will be contract
5. softskill required for the position
