I have gone through this phase of a developer. The steps i followed for the hand off is as follows:
1. Make a list of all the phases handled by you.
2. Emphasis to be laid on each phase with the activities carried out and how to go about it.
3. Emphasis to be laid on each phase or section's Pro's and Con's. If you have made any document on handling the errorneous situations for the same, do mention them.
I even made a list of steps to be followed before starting any development and the changes asked for by the company's SQA dept for next releases.

You make a document as follows:
Part 1.
The projects handled with status. Here you list down all the pending projects first and then list all completed projects. This helps your manager and the reliever to understand the situation better. You may also include the contact points of each project - internal and/or external.
Part 2.
Details of each project in detail
Project name - module - program name - location on the server - documentation details - status
Do not delete the official mails. Ask the administrator to take a back up of your mailbox.
Anyway, your Project manager will let you know more :-)
