Hi Shanti
NO issues. You shall submit Form 10 & 19. There is no issues with Bank Account number. Only thing is you should enclosed the cancelled cheque of that employee along with the form 10C & 19. It is better to enclose copies of Form 5 & 10 and Form 3A for the relevant periods.

Hi Madam,
You can get a letter from the employee stating that while at the time of joining i have given my nick name instead of original name as per govt.records. Also you should attach the school records or ration card xerox duly attested by you. Notary affidavit is not required. This letter with enclosures you can attach with the Form19 & 10C duly attested by you.
Thanks and regards.

Hi Madam,
Thanks. You can do one thing. At the time of resubmitting Form 19 & 10c, you please submit Form3a for all the years for that particular employee so that PF office will work superpast for early settlement.
Regards.
