It is heart wrecking accident. Feel highly sorry for the lives lost in the accident. Needs a thorough investigation by the concerned authorites to prevent such incidents occurance in future.

Thanks Raghu for sending the pictures of accident site of oxygen plant. There was hardly any information in the newspapers, which was expected. Having worked in a steel plant wanted to get some facts. The magnitude was tremendous. Details of real cause can be ascertained only after a thorough enquiry.

Thanks for the snaps. Just goes to show the importance of following safety procedures and implementing stringent maintenance schedules. The old "Chalta Hai" attitude followed by a lot of companies where safety is concerned has to change. Condolences to the bereaved.

Dear All,
Have you come across any reports on the cause of this tragic accident? There was a big hue and cry made about conducting immediate investigations from the ministers. Looks like public is again kept in dark about the actual reasons of this accident by this bureaucracy. Please share if you have any root cause analysis from your experience.
