People are basically graphic designers, copy writers, client servicing people etc.
This is an creative industry. keeping in mind pls share the appraisal form
and Secondly, Please share some good team building activities, which i can implement in my office ( creative world)

I think you can Implement 360 degree because in AD Agency work is done in a team so employees depend on each other a lot, second customers have direct impact from employee that develop the image of organisation so involve them in employee appraisal through feedback (use forms), it will in return even help in developing long term relationship with customer, for example mr. A from company X has given a project that involves your SEO then through his feedback for SEO on work and behavior( how employee dealt with him) can help you appraise the SEO but the important thing is the appraisal must not be based on the basis of how the last two or three client appraised the employee because sometime client even act like nerd, so maintain it in document form and analyse the average of all.
additionally you can customized it according to your own need for example mix it with KRA( Management by objective) method which is common because today flexibility is required you cannot stick to one universally structured method, so read and observe your organisation and its people, analyse how can you customize or modify a particular method or how you can use one method in a way that suits your organisation.

Forgot to tell you about team building activity, you can do games like dumbsherads, antakshri and make a team of people who have conflicts, you can also have movie sessions together like remember the titans and coach carter and invictus and then ask them to analyse and summarize it and how do they relate themselves to the story,these movies session inspire and motivates and might help employees to understand not only oneself but as well the others
