1.) If any one knows why frequency, severity rates are only calculated in millions per hrs and why not in billions or others.
2.) If any one know Safety Factor Calculating formula please tell me. I am facing lot problem due to this I have a general idea about Safety factor just line sf of Hoist n Lift is 12,Running rigger and slings is 8 and other purpose is 6 but formula of calculating SF is still not Known.

Dear Shashikant,
Incidence rates should use as denominator the average number of people exposed to risk (i.e. those persons covered by the injury statistics). Wherever possible, the number of hours worked by them, the number of hours paid for or failing that, the number of days worked by them, should also be used as a second denominator to calculate frequency rates for non-fatal injuries. For convenience, incidence and non-fatal frequency rates can be expressed per thousand persons and per million hours worked or paid for. I don't have any idea why they calculated in MILLIONS indeed of billions etc.,, hope our seniors will give a solution.

please tell me what is statutory requirement is to be fulfilled by Safety officer of perticular manufacturing unit regarding Safety and environment.
what paper should be submitted to DISH regarding safety and SPCB regarding environment it may be monthly,quarterly,Half yearly and Yearly.

Dear Shashikant,
Following reports needs to be submitted to DISH, PCB
Form 1 - watercess
Form V - Environment statement (every year beafore 30th sep)
Form- 4 Hazardous waste annual returns (every year 30 th jun)
Form -3 Hazardous waste register
Form-13 Hazardous waste manifest
Form-11 Lifting capacity testing through competent person (yearly)
Form-13 Pressure vessel testing (yearly)
Environment monitoring (Ambient air,Noise,Lux,ETP water,stack emission)
Consent to operate renewal as and when required, if new project then consent to establish,provisional Fire NOC
If MAH unit onsite emergency & offsite emergency plan to DISH
Safety report (After every 5 year)
Mock drill record (twice in a year)
Safety meetings MOM (quarterly)
etc.
