Hi Kavita,
The asset register is being maintained by the Accounts / Finance department in most of the companies or to some extent the Administration department.
In General the format is
Fixed asset Number / Name of the supplier/ Item description / Invoice No & Date / Quantity/ Department(allotted) / Location / Remarks
Verification Date:
Verified by: Name / Sign / Date
Signature of the User:
to some extent this may be useful to you. If you want you can add the cost part also & can keep space for change of location. The asset might have been allotted to one particular department at a particular location and some times due to exegencies / requirement we may be shifting the assest from one department to another department / one location to another location.
To have a good control a assest transfer note from also to be generated / duly approved and the necessary changes are recorded in the concerned asset form sheets and then only the assest need to be shifted.
At the time of any employee leaving the organization then along with the No due certificate the asset handing over and taking over formalities are to be completed and recorded accordingly.
