Hi
either you collect your insurance no from your previous company and the same you can give it to the new company where u have joined if that is not possible go for the new one by filling declaration form.
one minus point in fillin new declaration form is that you would not avail the contribution benefit of your previous company which you have contribute
regards
Mohan

Dear Navin ji,
It seems your company is deducting employer's share of ESIC & PF from your salary, which is totally wrong and not in accordance with Law.
Hope you are not making mistake in understanding the term salary and misunderstanding the term CTC.
Local Tax (PT) is not deducted as applicable in AP. I suppose, you are from Hyderabad.
