Good formats attached, but I feel exit interviews are best conducted face-to-face because this enables better communication, understanding, interpretation etc.,However, postal or electronic questionnaires are better than nothing, if face-to-face exit interviews are not possible for whatever reason.

Yes definitely.... After the exit interview form has been filled up by the employee, A one to one discussion is a must in which the HOD and a HR personnel have to be present.... Most of the times, they fill the form up just for the sake of doing it... But this discussion would make a great difference to the person who is leaving and also to the Organisation....
Based on the discussion, a detailed analysis can be made and sometimes there may be one odd cases where the person is leaving due to politicking by his HOD or his peers.... These issues would come out only when a face to face discussion takes place...
