Hi friends,
I need some basic info regarding PF
1) When a new employee joins an org, what is the name of the PF form that needs to be filled for him?
2) Similarly when an employee quits, what is the form to be filled for him to claim his PF
3) Would appreciate is could get a copy of the forms as well
4) Why is a new joinee asked for a form 16 from his previous organisation (for computation of TDS i presume??), what if he cant produce one??

1.At the time of joining, one has to submit Nomination cum Declaration (Form 2 revised)
After he has been allotted the PF account number, form for allotment of National Social Security Number shall also be submitted
2.When an employee leaves (infact after a waiting period of 2 months) he has to submit form 19 and form 10 C for withdrawing the PF accumulations. Alternatively, he can submit form 13 for transfering the accumulations to a new company.
3.All PF related forms can be downloaded from the website of Employees Provident Fund Organisation.
4. Form 16 of Income Tax pertains to tax deducted by the employer from the source salary) of the employee. If a new employer gets a copy of the form 16 of the new employee he can judge what exactly was the employee's salary with the previous employer and come to know whether the employee had stated any thing false about his present salary at the time of interview or application to this post.
I beleive that this is a bad HR practice to ask for form 16 issued by the previous employer. You are expected to beleive the employee and his statements and if lateron you come to know about any thing wrong, take action against him for such misstatement, but an employer has no legal right to ask for form 16.

Thanks Madhu Sir for Valuable information.
I think form no. 5 also required when employee joins the organisation with the form 2 (nomination)
And when employee leaves the organisation form no. 10 should also submitted towards the EPF organisation with other forms.

Dear Prashant,
Form 5 is also required. But form 2 is a declaration cum nomination by the employee duly countersigned by the employer whereas form 5 is a consolidated return by employer in which the details of all employees who join the PF during a month will be given. Similarly, form 10 relates to details of all employees who leave the establishment during the return month.
