The best way to improve your English is to follow the following steps and exercises:
- Start reading any Nationalized Newspaper. Read and underline the important words then try to remember their meaning in Telugu.
- Take a paragraph and read it aloud then rephrase it in your own words.
- Mirror Exercise: Stand in front of the mirror and speak in English, maintain the eye contact with yourself in the mirror.
- Its not at all difficult, just say to yourself that you are improving day by day.
- Count everyday as an accomplishment the day you are able to practice.

Nobody is a born communicator. You have potential with in you to improve. So as you have understood your area of improvement next step is to work on it. Accept your limitations. Worrying over it wont do any good to you. Participate in discussions and speak in english confidently even if you commit mistakes. Dont be shy or shouldn't feel fear about committing mistakes.
And partime please find out some good communication skill development course. Listen to English tv programmes and read English news papers.

It is all in the mind. What ever we say and what ever suggest, nothing is going to improve your English, unless u make up ur mind to break this jinx. Following few steps will definitely improve your mind set and the language.
1. Action kills fear. So instead of worrying about your weakness, start learning to talk in English, either by attending any English Speaking Class, or by keenly watching the other people's talking.
2. Take some of your good friends who is interested in improving your English. Make a mock presentation to him and ask for his opinion. But do it daily, till you gain confidence.
3. Buy good English comic books and start reading. It will improve your conversation capability.
4. Every day learn atleast 5 new words to improve your vocabulary. In the passing time u will acquire more knowledge.
5. Make a strong resolution not to loose heart. I once again emphasizing you that all is in your mind.
