1. Sir, eligibility for any benefit under ESI Act, 1948 and rules/regulations framed thereunder is determined by the appropriate Branch Office of ESIC which has issued the said Pehchan Card. The eligibility depend upon contribution period during which contribution paid and the corresponding benefit period. Therefore, it will be more better if you contact the concerned Branch Office of ESIC.
2. I think, on Pehchan Card, issued by ESIC, it is no where mentioned as to upto which period the said card is valid.

An employee who previously had esi, but last year she crossed the eligibility limit of 15000 and her esi was stopped but now as per new limit she is eligible what is the procedure to restart the esi scheme

1. Sir, you can use the same insurance number as was allotted earlier to said employee and start compliance w.e.f. the date the enhancement of wage ceiling to Rs.21000/- was made effective by the appropriate government.
2. For more guidance, I think, you can contact appropriate branch office of ESIC to which your unit is attached.
