I can understand your anxiety and can only assure you that all this is temporary. The only thing which you have allowed to happen is that you have permitted your past interviews to come and haunt you thus degrading your present. It can happen to anyone . So first and foremost, kindly dispel all fears from your mind that you lack in something and do not blame yourself for the same.
As suggested by all much more learned friends above, first is to get your confidence level up. Try doing this:
1. Read about any topic of your interest, prepare a gist
2. Now, when you are free in your room/place of stay, stand in front of a mirror, look yourself in the eyes and start talking about that topic. Keep your attention on the person looking back at you.
3. On the first go, you may find it a bit awkward, but after a few try's, you will hardly recognize the person looking back at you.
The above exercise will not only make you less self-conscious, but will give a tremendous boost to your confidence and improve your public speaking.
Once you have mastered this part, now start jotting down all probable interview questions and your answers related to your field. If you have a small recorder, feed in the questions one by one. Having done that, sit down at an appropriate time with the recorder and play the first question and stop. Start by answering it yourself. Continue till you have gone through all- with about 5 years of experience, you would be knowing most of the answers. For doubts, I am sure you can consult your friends, seniors etc.
There is no doubt that the more you practice, the smoother will be your flow. Gradually, start twisting these questions as you deem fit and get your answers- you will be surprised at your ingenuity.
In addition to what I have mentioned, follow the guidelines as given by the other members and you shall overcome all hurdles.

Hope you're now following the suggestions as given above by our expert friends. To add on, I'd also like to say, to be fluent/comfortable in English, you need to keep speaking English with your friends, collegues, family members, etc. whosoever you meet. Please do not bother/shy while speaking in English. After all practice makes a person perfect. So, please read & practice everyday moring & evening time whenever you're free; English grammer, newspapers/megazines(read loudly). Also try to write something which interests you in simple English, may be related to some story, your company admin policy etc. Listen carefully how your office collegues, friends, relatives, or other people speak so nicely & then you also try to speak in the similar manner. Forget about the tense & grammer you are using while speaking. You'll automatically improve all those things once you're are fluent & start speaking in English. After all, English is a language only. You must have some friends/collegues who can speak many languages. Do you know how they learned? Because they tried & practiced or esle, the community or the environment which they're brought up belong to the people speaking that pariticular language. Hence, you need to bring/create yourself that English speaking environment.

A lot of organisations have the practice of celebrating occasions in office in the form of employees’ birthdays, special days, festivals etc. While such initiatives do not have a long term impact on employee motivation but they surely work very well on a short term basis. , we all know that work place does create a monotony at times as most of us required to do similar jobs/assignments/work on a regular basis. Most of the employees visit the same work desk, switch on the same laptop/computer/use the same pantry and meet the same people over and over again at least five times a week! It can act as a refresher to break this monotony once in a while. HR may facilitate some changes every now and then to change some of these that may be changed.

Also try to speak only in English when you speak at home and among your friend circle, people my laugh at you, but learn to ignore them. This way you will develop confidence to use the language.
